Media Hold Request Form

Hold Audio/Visual Material(s) for a Class Viewing

Important Policy Information:
1. Faculty can place a hold on media item(s) up to one month in advance. Requests made less than one week in advance may not be able to be accommodated on the desired date.

2. For this type of request, the loan period for Faculty is two weeks. If you know in advance that you'll need a longer loan period, please make sure to indicate this in the additional comments at the bottom of this form. It is strongly recommended that faculty make their reservation(s) at least one to two weeks in advance so the library can attempt to recall any items that are checked out.

3. Hold requests will processed as soon as possible. The faculty member will be notified by email when the media material is available for check-out.

4. Media holds for classroom use will be retrieved from the shelf two days before the date needed. They will be held at the Library’s Media Desk in Periodicals to be checked out by the instructor. Valid ID cards are necessary in order to check out any material.
